QR code for fault reporting

Build a simple reporting point next to a device: a QR code that opens a short form.

Level: SimpleTime: 20–40 min

What you will build

A QR code placed next to a device or zone that opens a short reporting form. Treat this as a process experiment, not as a replacement for a full maintenance-management system.

What you need

  • the form URL,
  • a printer or a way to order a sticker,
  • a person or channel that will actually receive the reports,
  • about 20–40 minutes for the first version.

1. Keep the form short

At the beginning, three fields are enough:

  1. What does the report concern?
  2. What is happening?
  3. Photo or contact details — optional, depending on the process.

Do not collect data “just in case”. The longer the form, the less likely people are to use it.

2. Generate the QR code

The project contains examples/qr/generate_qr.py. After installing qrcode, run:

python examples/qr/generate_qr.py "https://your-form.example"

The script saves a PNG ready to print.

3. Test the flow

Scan the code as a guest or employee. Send three test reports. Check that all of them reach the right person and contain enough information to understand what happened.

4. Only then print more

Start with one device. If the process works, expand it. If nobody reacts to the reports, adding more QR codes only makes the underlying problem larger.

Important: the QR code itself is not the process. Value appears only when the report has an owner and a clear next step.
